Application Checklist
Prior to submitting the admission application, please make sure the following sections have been completed (including parent and student signatures where applicable) and all fees are attached.
Application Fees (Registration, Matriculation, Technical Fee, and Entrance Testing, if applicable)

Parent Information Section
Financial Commitment Section - signed by both parents, if applicable
School/Church References
Recent Photo (paper clip to application)
Health (Original HRS 680 and HRS 3040 Forms), Emergency/Medical Release Form
Original Birth Certificate (ALCA will make a copy from original BC)
Original Social Security Card (ALCA will make a copy from original SS)Ethnicity Choices - The selections are based on the U. S. Department of Education’s options we document for each child.

Please fill out one of the above 7 ethnicity choices on the Admissions Application for your child(ren).
An application is considered incomplete and cannot be processed, if all information is not filled out. Registration, matriculation, technology fee, and testing fee, if applicable, must be attached to the application.
Once your application is complete, it will be processed and testing, if needed, will be scheduled. When reference forms and test scores are received, the parent or guardian will meet with an administrator by appointment.
